It is not generally accepted that organophosphorous chemicals induce delayed pulmonary toxicity in some cases. We describe a rare case of organophosphate poisoning complicating delayed respiratory failure in this report. A 37-year-old male with a history of emotional depression was admitted to our ward after an ingestion of organophosphorous compounds, ”Malathion”. After an acute cholinergic symptom, he developed respiratory failure with respiratory index as low as 270 on day 7. He needed respiratory intensive care for seven days. He was referred to psychiatrist for mental care on day 22.
