Sequential changes of intra-brain tissue concentration of Etoposide were investigated to make clear the optimal method of administration for malignant glioma. The maximum concentration of Etoposide reached 1μg/g, whether it was given via intravenous route or intraarterial route, on condition that blood brain barrier was opened with intraarterial administration of mannitol. With half doses of Etoposide given via intraarterial route, intratissue concentration reached equal to or more than what was given via intravenous route. In clinical situations, chemotherapy of malignant glioma has been unsatisfactory so far. One of the key for success is how to reach the peripheral part in the tumor, where the blood brain barrier is not destroyed but tumor cells exist. Etoposide, the new chemotherapeutic agent with unique action mechanisms, should be tried with patients of malignant glioma after blood brain barrier opening with intra arterial injection of mannitol.
