The form of death certificate has been revised since 1995 in Japan. We have summarized the revision and the issues which has needed the improvement so far. The certificate determines the cause-death relationship and the responsibility of the person who might give the cause. The doctor who writes the form ought to judge whether he must inform the police of the possibility of crime or of the abnormality in the death. The ”initial” cause of death should be described properly in the certificate and the category of the cause of death must be selected in terms of the ”initial cause of death”.
