We report a histological and immunohistochemical study of a case of Bellini duct carcinoma (BDC) originating from the collecting duct epithelium of kidney. A 66 year old man had operation for lung cancer (histological type: adenosquamous carcinoma). During his condition after the operation, a right renal neoplasma was found. As suspected metastatic carcinoma after clinical examinations a right nephrectomy was performed. Macroscopically, it was a well-circumscribed encapsulated mass 2×2.5×2cm in size and its cut surface was grey-white. The tumor was localized mainly in the medulla of the upper pole and extended into the cortex, but did not invade the renal pelvis. Histologically, tumor cells showed a papillo- tubular pattern with fibrovascular stroma and rare psammoma bodies. The tumor cells showed negative reactions to PAS(Periodic acid Schiff) , alcianblue and Sudan III. Immunohistochemically, they showed positive reactions to EMA(Epithelial membrane antigen) , SBA(Soybean agglutinin) , DBA (Dolichos biflorus agglutinin) and PNA (Peanut agglutinin) , but negative for CEA (Carcinoembryonic antigen) , LTA (Lotus tetragonolobus agglutinin) and THP(Tamm-Horsfall protein) by LSAB (Labelled Streptavidin Biotin) method. These histological and immunohistochemical findings revealed that the renal neoplasma is neither lung cancer nor renal cell carcinoma. With the results of macroscopic, histological and immunohistochemical studies, we diagnosed this renal tumor as BDC originating from the collecting duct epithelium.
