A 24-year-old man was admitted with a six-month history of fever. On admission, a solitary large mass was detected in the liver, along with multiple nodules in the bilateral lungs. At first, various studies suggested a liver abscess and septic embolism of the lung. However, histological examination of the liver mass and pleural effusion indicated a diagnosis of T cell malignant lymphoma classified into the diffuse large cell type. Chemotherapy was performed, but the patient died without it's effect. There is no doubt that it is difficult to confirm the primary focus of stage IV lymphoma. However, we suspected that the correct diagnosis in this case was primary hepatic lymphoma, because a solitary large mass was seen and it's histological feature was compatible with T cell type primary hepatic lymphoma with extensive hemorrhagic necrosis. Although primary hepatic lymphoma is extremely rare, this disease should be kept in mind in differential diagnosis of a patient with a liver tumor.
