The pancreas has a reserve capaciy on the exocrine function and the diagnosis of pancreatic disease couldn't be always made only by the exocrine pancreatic function test and serum amylase value. The change of serum amylase level in the Vagostigmin test and the Pancreozymin-Secretin test (P-S test), has been observed. But, it had a small clinical significance because of the minor response. Author carried out a provocation test using Caerulein (0.1μg/Kg, intramuscularly) in the cases of pancreatic diseases and liver diseases, and evaluated amylase, P-amylase, lipase, elastase I, pancreatic secretory trypsin inhibitor (PSTI) and CEA. The criteria of the positive provocation test was the 15% increase after provocation. Cases with more than 25% increase were also investigated. The Caerulein provacation test with the many kinds of examinations was thought significant as a screening test in case of the impracticable P-S (or C-S) test and the pancreatic hypofunction.
