    This paper presents a case o: sarcomarcid renal cell carcinoma discoverd on treating rupture of hydronephrosis. The patient was a 58-year-old woman who underwent emergency nephrectomy following image diagnosis of rupture of the light hydronephrosis. The size of the extracted kidney approximated that of an infant head owing to hydronephrosis. The point of rupture was the lower pole. Although no neoplasnia lesion could be clearly detected macroscopically, histologic examination disclosed carcinoma located mainly in the medulla, extending from the renal pelvis to the subcapsula of the cortex. The tumor was comprised of epithelial and sarcomatous cells, both containing glycogen and lipid components. Immunohistochemically, anti-EMA(Epithelial membrane antigen), vimentin, and PNA (Peanut agglutinin) sera reacted positively with all neoplastic cells, as showen by LSAB(Labbeled Streptavidin Biotin ) method. The present tumor thus appears quite likely to be sarcomatoid renal cell carcinoma which originated from the epithelium of the distal tubule.
