In the prognostic multiparameter analysis of 79 patients with meningiomas, many possible factors such as proliferating cell nuclear antigen (PCNA) and tumor histology relating to the recurrence of meningioma were studied. The median follow-up time for all patients was 60 months. Tumors were surgically extirpated and specimens were embedded in paraffin. The PCNA positive ratio (PR) ranged from 0.3 to 27.8%. The values of PCNA PR in recurrent meningiomas were generally higher than those without recurrence. When the clinical courses of the patients were compared, those with PRs under 2.0% were significantly higher recurrence-free ratio than those with PRs over 2.0%. Uniparameter analysis demonstrated that PCNA PR, tumor histology and extent of resection were significant independent variables influencing recurrence. Age, sex, tumor size, tumor location, and tumor stainining were not significant factors. The multiparameter analysis including PCNA PR, tumor histology, extent of resection and radiation were examined. The value of the multiple correlation coefficient between recurrence and multiparameter factor was 0.667 and it was significant. Total correct rate of recurrence with multiparameter factor was 91.8%. Total correct rate in the analysis including PCNA PR proved to be higher than that with the exception of PCNA PR. The prognostic multiparameter analysis may be clinically useful for planning of adjuvant therapy in patients with meningiomas. PCNA-immunoreactivity is the most important factor affecting the recurrence.
