Fifty-two patients with post-operative maxillary cysts (on 55 sides), who underwent surgical treatment at our department from 1991 to 1995, were analyzed with respect to the operative method used before and after the introduction of the rigid endoscope. Also, the indications for endonasal endoscopic intervention were reviewed based on the findings of pre-operative imaging and those of surgery. The results were as follows: 1. All 45 patients (48 affected sides) who were treated before endoscopic surgery was introduced underwent maxillotomy through a gingival approach. Among 7 patients who were treated after endoscope introduction, five (71.4%) were treated by endoscopy via an endonasal approach, and the remaining two underwent trans-gingival maxillotomy. 2. As for solitary cysts, the indications for endoscopic endonasal surgery were considered to depend on the size and location of the cysts. Many cysts can be successfully treated by an endoscopic endonasal approach using optimal instruments, with the exception of relatively small cysts located laterally (far from the nasal cavity), anteriorly, or superiorly (near the orbita) in the maxilla. We conclude that endoscopic endonasal surgery is the first choice of treatment for many post-operative maxillary cysts, after an accurate diagnosis as to size, location, and the number of cysts, with the help of imaging devices such as computed tomography.
