A study on the detection of mycobacteria and clinical backgrounds of patients with mycobacterial infection from 2005 to 2015, at the yamaguchi grand medical center

山口県立総合医療センターでの抗酸菌感染症の状況を明らかにするため,2005年4月から2015年3月までの10年間に抗酸菌検査を行った全1965検体を対象に,抗酸菌検出状況,患者の年齢分布と性別,患者の臨床背景について検討した.塗抹,培養,PCR検査のいずれかが陽性になった検体は177件であり,陽性率は9.0%であった.検出された抗酸菌の内訳は結核菌が28.2%,非結核性抗酸菌(NTM)が71.8%であった.NTMの93%はMycobacterium avium complexであった.結核は60代以上の男性に多く,80代が最多であった.一方,NTM感染症は50代以上の女性に多く,70代が最多であった.結核50例のうち,塗抹,培養,PCR検査が同時に実施された30例について,塗抹,培養,PCR検査の結果を比較検討したところ,塗抹陰性かつPCR陰性かつ培養陽性例が13例と最も多く,培養検査の重要性が再認識されるとともに,当センターの結核患者には培養検査のみでしか菌が検出できない,いわゆる診断困難例が多いことが分かった.今後も当センターにおける調査を継続することは抗酸菌感染症の状況を把握していくために重要である.さらに,山口県全体の状況を明らかにしていくためには,県内の他施設において同様の検討がなされることも望まれる.

To evaluate the long-term trend in mycobacterial infections in our hospital, we examined the detection of acid-fast bacilli, age distribution, sex differences, and clinical backgrounds of patients from whom 1,965 specimens were tested for acid-fast bacilli during a 10-year period between April 2005 and March 2015. The positive rate of acid-fast bacilli tests was 9%. Of the isolates, 28.2% were Mycobacterium tuberculosis(TB),and 71.8% were nontuberculosis mycobacteria(NTM)
