Hepatectomies for the Hepatocellular Carcinoma with Special Reference to the Systematic Subsegmentectomy

Seven hepatectomies for the hepatocellular carcinoma were performed at the 2nd department of surgery of Yamaguchi university hospital from 1983 to october 1985. Six patients had relatively small cancers (maximum diameter : 1.8~4.0cm) which complicated the liver cirrhosis. The liver function was assessed preoperatively by ICGR15. ICGR max, OGTT pattern. Systematic subsegmentectomies or segmentectomies were performed in 6 cases with liver cirrhosis. Right two segments were resected in huge tumor case without liver cirrhosis. Ultrasonography and CUSA were used during the operation. Pre― and postoperative managements were done according to the method in the National Cancer Center. No cases developed liver failure postoperatively and postoperative course was good. All patients except one with many doughter nodules were survived. Systematic resection of the subsegment or segment seemed to be appropriate for the operation of the hepatocellular carcinoma with liver cirrhosis.
